About swim kids & agreement
I, the parent/guardian of the kid, hereby give my permission for my child to participate in the swim kids swimming school program.

Photo release
Happy Swimmers Swimming School is hereby granted permission to use photographs and/or video of my child/children in marketing materials (Social Media platforms and others)

Waiver release statement
For in consideration of Happy Swimmers School. As the service provides for this program, we agree to release and forever discharge Happy Swimmers Swimming School officers and employees from any and all liabilities, demands of claims for loss and damage which may be sustained on account of the participation in this program.
